Service Coordination Committee
This committee addresses service coordinators who provide information on community resources, referrals to other programs, facilitating the development of family goals (IFSP), developmental screenings, evaluation of children to determine eligibility, and coordinate transition to other programs and services. Family Support: This committee will look at the information being provided to families during home visits, identifying and establishing a medical/health home for families, how health and physical assessments of newborns and mothers are being done and ongoing home visits as determined by the families’ needs. Public Relations: This committee will design and implement public awareness initiatives that make primary referral sources aware of the Help Me Grow Program. Activities may include, but are not limited to, the following: writing newspaper or magazine articles and ads; coordinating a community events calendar; advertising; making public service announcements, commercials, videos or web site testimonials; planning publicity for HMG events; determining and ordering promotional materials for community events; and disseminating public materials that target the medical community and families.
