The KIT is an organizational and informational tool designed for families receiving Help Me Grow services.
It’s hard enough for families of young children to keep their lives organized. Throw in extra stressors and it’s
almost impossible. The KIT is designed to help families get some control back by getting organized and
receiving information about the resources available to them.
The KIT is free to families receiving Help Me Grow services. It includes a file folder box, file folders for
personal and medical information and other various services you may be receiving and other resources
you may be interested in.
The following are the titles of the folders in the KIT:
- Welcome to Help Me Grow: Explains the 3 programs offered within Help Me Grow and the various standing committees for parents who are interested in learning more and getting involved on a committee level.
- The I.F.S.P.: Explains the importance of the IFSP (Individualized Family Service Plan) and your family’s role in the IFSP process. A place to keep your copy of your IFSP to review and know when your updates are.
- Websites, Resources and other Information: Tips for families on letter writing and record-keeping, federal and state phone numbers, addresses and web-sites for early intervention and preschool age services, how to find and evaluate internet health sites and lots of internet website addresses for every parenting need.
- Financial Resources: Explains the different financial resources available to families including state and local phone numbers.
- Family & Medical History: Worksheets include forms for developmental history, family health history, insurance information, child’s birth and shot records, records for illnesses, surgeries and medications, a list for service providers, a checklist to keep track of any food allergies or special nutrition needs and a blank calendar to keep track of daily activities in the family.
- Official Documents: A closed pocket folder for copies of birth certificates, social security cards, medical cards, court or custody papers, approval letters from DJFS or WIC,,,,,
- Developmental Activity Ideas: Appropriate activity ideas for parents to do with their children.
- Hearing / Audiological Information: Explanation of what an audiologist does and a file to keep all evaluations and reports.
- Occupational Therapy Information: Explanation of what an occupational therapist does and a file to keep all evaluations and reports.
- Physical Therapy Information: Explanation of what a physical therapist does and a file to keep all evaluations and reports.
- Speech / Language Information: Explanation of what a speech / language pathologist does and a file to keep all evaluations and reports.
- Vision Information: Explanation of the preschool vision screening and a document about visual impairments.
- Other Evaluations: Can include any evaluations and reports that do not fit in previous folders.
- Transition from Help Me Grow to Preschool: Explains the transition process and has a time-line so you know what steps are coming up. Includes tips on how to select a preschool for your child and a list of the preschool contacts for Summit County schools.
- Information about the MFE: Walks parents through the Multi-factored Evaluation (MFE) process and explains the areas of assessment in an easy to understand format. There is a separate file folder for a copy of your child’s evaluation.
- Information about the IEP: Explains the Individualized Education Program (IEP) conference, how the IEP is written, the parent’s role in the process and tips on how participate effectively and be their child’s best advocate. There is a separate file folder to keep a copy of your child’s IEP.
- Diagnosis Information: A folder to keep any information that parents have collected in regards to their child’s diagnosis.
